When I try to boot a machine that has secure boot turned on, I never see the PESE flash drive (I've tried 7, 8.1 and 10 builds).  Is there a way to make a bootable flash drive that will work with secure boot enabled?

Secure Boot is a feature of UEFI. You need to go into the UEFI settings and disable Secure Boot. Don't need to delete security keys but disabling is good enough. On my Surface 3 this is a setting I access by holding the volume - button at startup and releasing it when the Surface logo pops up. For what it's worth, when I start PESE it's from a chipdisk on a full size USB adapter and it starts automatically. This is from changing another setting in the UEFI to rearrange the boot order.

I have gotten around this issue ONCE with Windows 8.1 as the source under Win8.1PESE. The same should be possible under Win10PESE. They should be x64 builds only.

The building should work on most UEFI (by using same Arch +Fat32 usb format) or Legacy Bioses computers without changing anything except only the boot order.
It can depend on the tool used to create your USB stick, disk. For example, the good rufus does not allow the double UEFI/Bios boot, saddly.
You can try with 1 of the 2 "Copy to USB-Device" plugin or with USB7Ice. It should work, normally.
